Empower Nutrition Counseling welcomes both self-pay and insurance clients. You'll find the in-network insurance providers listed below. Keep in mind that in-network status doesn't always mean your visits will be covered — I suggest calling your insurance company to confirm your benefits ahead of your first appointment.

• Does my plan cover outpatient nutrition therapy CPT codes 97802 (medical nutrition therapy assessment), and 97803 (medical nutrition therapy follow up)?
• Do I have any nutrition counseling visits covered under the ICD-10 Code: Z71.3?
• Do I have any nutrition counseling visits covered under my specific eating disorder diagnostic code? (This can be obtained from either your therapist, primary care provider, or psychiatrist).
• What is my current coverage for telehealth appointments?
• Do I need a referral from my physician?
• Do I have a deductible for nutrition counseling services? If yes, how much is it and how much has been met?
• Is there a copayment or coinsurance? If yes to either, what is the cost?
• How many sessions are covered per year?

If you do not have insurance or if I am not in network with your insurance company, I offer an out-of-pocket rate of $150 per hour for initial appointments and $130 per hour for follow-up appointments. You will be charged at the time of service.
